Job Summary
Pahrump Health and Rehabilitation Center is currently seeking a
Wound Care Nurse (LPN or RN)
to provide specialized wound care services and support optimal healing outcomes for our residents. This role is responsible for assessing, treating, and monitoring wounds, as well as collaborating with the interdisciplinary team to ensure high-quality, evidence-based care.
Essential Functions
Performs comprehensive wound assessments and develops appropriate treatment plans
Provides wound care treatments including dressing changes, debridement (within scope), and infection prevention
Monitors wound healing progress and documents changes accurately
Collaborates with physicians, nursing staff, and interdisciplinary team members on care plans
Educates staff, residents, and families on wound care protocols and prevention strategies
Ensures compliance with infection control standards and regulatory guidelines
Maintains accurate and timely documentation in accordance with facility policies
Participates in Quality Assurance and Performance Improvement (QAPI) initiatives
Assists with skin rounds and prevention programs (e.g., pressure injury prevention)
Other duties as assigned
Education, Licensure, and Experience
Graduate of an accredited school of nursing (Required)
Current
RN or LPN license
in the state of employment (Required)
Wound care experience in long-term care or rehabilitation preferred
Wound Care Certification (WCC, WCN, or similar) preferred but not required
Strong knowledge of wound care protocols, infection control, and skin integrity management
