Advanced Practice Provider – MaineHealth Geriatric Medicine, Farmington, Maine
Position: Advanced Practice Provider (Physician Associate / Nurse Practitioner)
Location: MaineHealth Franklin Hospital, Farmington, Maine
Schedule: Part-time, 20 hours per week (typically 2 days per week)

Responsibilities
Provide primary care services to patients in the hospital and nursing home setting, including assessment, diagnosis, prescribing, treatment, and patient education. Work closely with physicians and other clinicians in a team approach to patient care, focusing on post‑acute and long‑term care patients. Participate in a shared patient model with geriatricians and other APPs in partnered facilities (long‑term, assisted living, and skilled rehabilitation care settings) in Farmington, ME.
Minimum Qualifications
Successful completion of an accredited Nurse Practitioner or Physician Associate/Assistant program.
Two years of Nurse Practitioner or Physician Associate/Assistant experience preferred but not required.
Current license issued by the State of Maine or eligibility for licensure.
Current BLS certification.
