Ideally 2 years’ experience in an advanced practice role, preferably with a neurology patient care setting.The Neurology Advanced Practice Provider (APP) service will operate as a **dedicated, 7- days-per-week service line** embedded exclusively within the Neurology Stroke program. All APPs will rotate between two defined assignments: the **Inpatient Neurology Team** and the **Acute Stroke Consult Team.** The APP serves as a key clinical provider across the inpatient neurology service and acute stroke consult team. This role combines independent patient management, rapid-response stroke care, and continuity across care transitions. The APP works collaboratively with attending physicians and residents while providing clinical consistency, protocol expertise, and high-quality stroke care.Inpatient Neurology Care Team* Manage an independent panel of admitted neurology patients in collaboration with attendings and residents* Perform daily assessments, documentation, and participate in interdisciplinary rounds* Initiate and manage secondary stroke prevention therapies per protocol* Monitor for and escalate changes in neurological or medical statusCare Coordination & Transitions* Provide continuity across resident handoffs and ensure consistent care delivery* Coordinate discharge planning, follow-up appointments, and interdisciplinary communication* Ensure completion of discharge documentation and tracking of pending resultsAcute Stroke Consult Team* Respond to stroke alerts (Code Stroke) and assist in rapid evaluation and treatment decisions* Perform NIH Stroke Scale assessments and support thrombolytic/thrombectomy workflows* Guide residents on protocols, documentation, and time-sensitive benchmarks* Serve as a clinical and procedural resource during acute stroke eventsLongitudinal Consult Follow-Up* Maintain and manage a follow-up list of stroke consult patients with ongoing needs* Conduct reassessments, adjust care plans, and address evolving clinical issues* Document billable follow-up encounters and support continuity post-consult* Act as a stroke resource for primary teams after consult sign-offStroke Education and Prevention Counseling* Initiate etiologic workups and ensure guideline-directed therapy* Track pending diagnostics and ensure appropriate follow-up ownership* Identify high-risk patients requiring attending review**Qualifications**Nurse PractitionerRequire a Master's degree as a Nurse Practitioner, an active New York State Registered Nurse license in good standing, an active New York State Nurse Practitioner licensure in good standing and current national certification as an Nurse Practitioner.Physician AssistantRequired completion of an ARC-PA approved Physician Assistant program, an active New York State Physician Assistant licensure in good standing and current national certification as a Physician Assistant by NCCPA (National Commission on Certification of Physician Assistants)NPs and PAs will demonstrate clinical competence, superior verbal and written communication skills, interpersonal effectiveness and customer relation skills, computer competence and must have an active desire to learn.
