RN Case Manager: In-Home Wellness & Care Coordination

Hispanic Alliance for Career Enhancement is seeking a Nurse Case Manager in Fairfax Station, Virginia. In this full-time role, the RN will assess and coordinate case management activities, using clinical judgment to address health and social factors while working directly with members. Candidates must have an unrestricted Virginia RN license, 3+ years of clinical experience, and be able to travel 75%.
