Role Overview
We’re seeking an experienced
Physical Therapist – Home Health
to deliver personalized, high-quality care across a diverse and scenic Northern California service area. You’ll support patients recovering and regaining independence in their own homes—making every visit meaningful and impactful.
WHAT YOU’LL DO
Perform comprehensive patient evaluations and develop individualized treatment plans
Deliver evidence-based physical therapy interventions in home settings
Establish and manage care and discharge plans
Maintain timely and accurate clinical documentation
Supervise therapy assistants as needed
Collaborate in an interdisciplinary care model to optimize patient outcomes
Requirements
Doctor of Physical Therapy (DPT) – required for licenses issued after Jan 2016
Active Physical Therapy license (CA and/or applicable states)
Current CPR/BLS certification
Valid driver’s license
Home health experience preferred (1+ year ideal, but willing to train motivated candidates)
