Working independently while reporting to a Registered Nurse (RN), this role ensures high-quality patient care through vital sign monitoring, medication administration, wound care, and comprehensive documentation. The LPN actively participates in care planning and patient education, collaborating closely with the healthcare team to support patient health and recovery in their home environment.
Responsibilities
* Provide direct patient care tailored to individual needs
* Administer medications accurately and safely
* Monitor vital signs and report changes promptly
* Perform wound care and assess healing progress
* Document patient interactions and care activities thoroughly
* Develop and contribute to patient care plans
* Offer health education to patients and families
* Communicate effectively with patients and healthcare team members
* Collaborate with interdisciplinary teams to coordinate care
Required Qualifications
* Entry-level experience in practical nursing
* Licensed Practical Nurse (LPN) diploma or equivalent
* Active LPN license in Massachusetts
* Valid Massachusetts Driver's license
Preferred Qualifications
* Experience in patient assessment and infection control
* Knowledge of medication management and Electronic Medical Records (EMR)
* Basic Life Support (BLS) certification
* Strong communication and time management skills
* Demonstrated empathy in patient care
