Job Title: Home Health - PRN - Per Visit - Registered Nurse | Location: Irving, TX 75014 | Sign-on Bonus: $2,500
Description
Our Registered Nurse (RN) will have a variety of duties that help the company thrive and manage continuity of care. This candidate must maintain 32 productivity points weekly to qualify and participate in the compensated on-call program. This RN will see visits within the scope of practice of their license, including OASIS visits, skilled nursing visits, and supervised visits. They will also participate in the compensated perception of new hires. Administer skilled nursing care for patients of all ages in their place of residence and coordinate care with the interdisciplinary team, patients, families, and referring agency. Assumes responsibility for, and participates in, the coordination of care.
Responsibilities
Case management and supervised visits.
Perform skilled and comprehensive assessment of the patient during the initial admission.
Assess and chart observations of the patient’s condition at each visit (vital signs).
Provide skilled nursing care as needed in accordance with the plan of care (administering medication, cleaning/dressing wounds, assessing healing progress).
Provide proper patient education for home health care procedures and strategies.
Responsible for overall clinical record maintenance to include visit notes (must be completed within 24 hours), comprehensive assessments including OASIS (must be completed within 48 hours), medication profile, and orders/plans submitted in a timely manner.
Maintain confidentiality of patient PHI in accordance with HIPAA.
Keep the patient comfortable through palliative care.
Qualifications
Current Texas license as a Registered Nurse.
2+ years of experience as an RN in Home Health.
Current CPR certification.
Reliable transportation.
Texas Driver's License and Auto Liability Insurance.
Current TB skin test.
EMR experience (Kinnser preferred).
