Certified Home Health Aide (CHHA) – Full-Time | Field-Based
Vitality Hospice & Palliative Care is seeking a
compassionate and dedicated Certified Home Health Aide (CHHA)
to join our growing team. This is an opportunity to work for a
clinician-owned and operated hospice

Position Details
Schedule:
Full-Time, Monday–Friday, typically 7:00 AM–3:00 PM
Occasional evening/weekend overtime available
Field-based position with travel required (mileage reimbursed)
Active CHHA certification required
What You'll Do
Working under the supervision of a Registered Nurse Case Manager, you will provide essential hands‑on care and support, including:
Assisting with mobility, transfers, and ambulation
Providing personal care (bathing, grooming, dressing, feeding)
Offering companionship and emotional support
Performing light housekeeping tasks
Documenting care accurately and in a timely manner
Supporting a culture of compassion, quality, and respect
You will be an integral member of our interdisciplinary team focused on clinical excellence and patient‑centered care.
What We're Looking For
Active
Certified Home Health Aide (CHHA)
certification
Valid driver's license, reliable vehicle, and current auto insurance
Compassionate, reliable, and self‑directed
Strong communication and time‑management skills
Ability to work independently and follow care plans
Commitment to serving patients with chronic, serious, or terminal illnesses
