Experience: 1 year
Licenses: OT-IL (Copy needed)
Certifications: OTR-National Board for Certification in Occupational Therapy (Copy needed); BLS-AHA (Copy needed)

DOB Required
Work History: Should not be more than 90 days of gap in last 1 year
Position is for NICU/PEDS, so NICU/PEDS experience is required!
