OVERVIEW
The Primary Care Provider (Nurse Practitioner or Physician Assistant) delivers high-quality, patient-centered care within a multidisciplinary outpatient setting. This role focuses on acute care, preventive services, and longitudinal primary care in a collaborative environment alongside musculoskeletal and mental health providers.

This position is ideal for a clinician who is adaptable, service-oriented, and comfortable practicing in a modern, team-based care model that emphasizes access, efficiency, and a high-touch patient experience.

KEY RESPONSIBILITIES
CLINICAL CARE DELIVERY

Provide comprehensive primary and acute care services, including evaluation, diagnosis, and management of common medical conditions

Conduct preventive care visits, annual exams, and health screenings

Manage chronic conditions within scope and clinic capabilities

Order and interpret laboratory tests, imaging, and other diagnostics

Prescribe medications in accordance with New York State regulations

High touch, low volume schedule

PATIENT EXPERIENCE

Deliver a personalized, patient-centered care experience with strong attention to service and communication

Ensure timely access to care, including same-day or next-day visits when appropriate

Build and maintain strong patient relationships, supporting continuity and proactive care

Provide clear guidance, education, and follow-up to support patient outcomes

CARE COORDINATION & REFERRALS

Collaborate closely with internal providers across disciplines, including musculoskeletal and mental health services

Facilitate referrals to external specialists and diagnostic services as needed

Coordinate follow-up care, including tracking results and supporting care plan adherence

CLINICAL OPERATIONS

Maintain accurate, timely, and compliant documentation in the electronic medical record

Adhere to established clinical protocols, workflows, and compliance standards

Participate in quality improvement initiatives and clinical program development

Support clinic operations, including coverage and workflow optimization

TEAM COLLABORATION

Partner with multidisciplinary clinical team to ensure consistency and quality of care delivery

Participate in team meetings, case reviews, and interdisciplinary collaboration

Contribute to a professional, team-oriented clinical culture

QUALIFICATIONS

Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in New York State

Board certification required

Minimum of 3–5 years of clinical experience in primary care

Strong clinical judgment with the ability to manage a broad range of conditions

Excellent communication and interpersonal skills

PREFERRED EXPERIENCE & SKILLS

Experience in a multidisciplinary or team-based care environment

Comfortable working in a high-service, access-focused care model

Strong organizational skills and attention to detail

Interest in preventive care, lifestyle medicine, and holistic care

Pay commensurate with degree and experience. Range ($130,000-$200,000)
