Licensed Practical Nurse
The LPN is responsible for assessing, planning, implementing and evaluating the care of adolescent patients with medical and psychiatric illness, both acute and chronic.
Responsibilities
Assess, plan, implement and evaluate care for adolescent patients with medical and psychiatric illness (acute and chronic).
Seek methods to improve patient care on an ongoing basis; demonstrate knowledge of regulatory law as it relates to the care of the adolescent patient.
Perform other duties as assigned as needed.
Administer physician-prescribed medications and vaccines.
Collaborate with other members of the medical team to maintain and order adequate inventory of medications and medical supplies.
Respond to medical emergencies, provide first aid, and medication treatment.
Coordinate with the RN to designate appropriate client referrals to contracted physicians and other medical agencies or practitioners; establish LPN role in clinic, reporting hierarchy, and process.
Coordinate with community service agencies to ensure routine and emergency medical care for clients and team members; communicate professionally with outside partners.
Promote and educate good health habits, proper hygiene, and good nutrition; include education for individual and group settings.
On-call: participate in a rotating schedule to provide emergency on-site assistance as needed.
Maintain medical documentation, medical history, and files in accordance with regulations and HIPAA; document changes in the EHR system.
Participate in nursing-related performance improvement activities.
Teach and participate in health education classes for clients using approved curricula addressing health topics, sexuality, hygiene, nutrition, and diet.
Collaborate with the RN to develop medical portions of the individual treatment plan (RN leads the overall plan with LPN collaboration).
Participate in multidisciplinary treatment team meetings as needed.
Participate in Safe Crisis Management training and periodic recertification; complete mandatory training and meetings as scheduled.
Maintain compliance with company exposure plans, including OSHA and blood-borne pathogens.
Identify opportunities to improve patient care and stay informed of regulatory requirements related to the care of the mentally ill patient.
Designate appropriate client referrals to contracted physicians and other medical agencies as needed; coordinate with community service agencies to ensure routine and emergency medical care.
Administer medications and vaccines as prescribed; promote healthy habits and nutrition for clients.
Maintain inventory of medications and medical supplies; ensure documentation and HIPAA compliance.
Participate in health training for clients, including hygiene, nutrition, and diet; participate in multidisciplinary treatment team meetings.
Hiring Requirements
Current licensure as an
LPN
in Colorado.
Must be 21 years of age or older.
Must pass physical and pre-employment drug/alcohol screening.
Must pass criminal background checks, including Child Abuse & Neglect Databases; pass federal and state background checks for specified crimes.
Must possess a valid Colorado Driver’s License.
