Home Base Medical Care , part of the Optum family of businesses, is seeking a Nurse Practitioner to join our team in Nash, Edgecombe, and Wilson Counties, NC. Optum is a clinician‑led care organization that is changing the way clinicians work and live.
You will be an integral part of the Optum Care Delivery team, delivering care to patients in their homes. Leveraging the clinical resources, data, and support of a global organization, you will help patients live healthier lives and contribute new practice ideas and initiatives that could improve care for millions of patients.
Primary Responsibilities
Conduct in‑home assessments on health plan members, including past medical history, review of symptoms, physical examination, medication review and depression screening.
Check vitals and perform physical exams that include monofilament test, urine dipstick and foot exam (as appropriate).
Identify diagnoses for care management and active medical management to advance treatment.
Formulate a list of current and past medical conditions using clinical knowledge, judgment and assessment findings.
Communicate findings to inform the primary care provider of potential gaps in care.
Educate members on disease processes, medication and compliance.
Comply with all HIPAA regulations and maintain security of protected health information (PHI).
Required Qualifications
Current, unrestricted Nurse Practitioner licensure in the State of Practice, or the ability to obtain by the assigned start date.
Certification through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
1+ year of clinical experience at the highest level of education or 3+ years of experience as a Registered Nurse.
Current, unrestricted Registered Nurse licensure in the State of Practice, or the ability to obtain by the assigned start date.
Reliable transportation to travel to client and/or patient sites within a designated area.
Ability to obtain DEA licensure/prescriptive authority post‑hire in applicable states.
Ability to spend at least one hour with a member in their home, even in understaffed or remote areas, and in the presence of pets or tobacco users.
Ability to lift a 30‑pound bag in and out of a car and navigate stairs and various dwelling conditions and configurations.
Preferred Qualifications
Experience in Gerontology, Cardiology, Internal Medicine or Endocrinology.
Home care or home visit experience.
Proven administrative and organizational skills and the ability to effectively communicate with seniors and their families.
Computer literacy and ability to navigate the internet.
Compensation and Benefits
Compensation for this specialty generally ranges from $104,500 to $156,000. Total cash compensation includes base pay and bonus and is based on several factors, including local labor markets, education, work experience and may increase over time based on productivity and performance. In addition to your salary, we offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution, all subject to eligibility requirements.
