Essential Functions
Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, and other medical care treatment.
Documents patient care given.
Monitors, records, and communicates patient condition as appropriate to the care team, physician, patient, and family.
Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral, and spiritual/psychosocial support needs.
Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.
Works as an advocate for the physical and emotional well‑being of the patient.
How You Will Contribute
A Registered Nurse (RN) who excels in this role:
Accurately performs patient assessments and identifies patient needs.
Identifies and initiates appropriate nursing interventions.
Provides care appropriate to condition and age of the patient.
Performs timely and appropriate documentation relating to medical necessity in the medical record.
Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
Performs timely and accurate QI assessments.

What We Are Looking For
Applicants should have a current state RN license.
Additional Requirements Include
Basic Life Support certification required within 30 days of hire.
One year of experience preferred.
Bachelor’s degree preferred but not required.
The registered nurse assesses, plans, implements and evaluates care for a given population based upon age‑specific and cultural needs. This is done in a collaborative multidisciplinary approach embracing quality and safety.
Assures quality of care by measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital philosophies and standards of care set by North Carolina State Board of Nursing and other governing agency regulations.
Coordinates the delivery of patient care for the Rehab Unit, facilitates problem solving with staff members and communicates necessary issues to nursing administration and the program manager.
