Responsibilities:
Learn and expand your clinical skills caring for clinically complex patients in the home
Perform 1-3 Start of Care visits per day, focusing on 1:1 care with each patient
Deliver 1:1 skilled nursing, intervention, and supportive care in accordance with individual care plans, including administration of medication, wound care, treatments, and comfort care
Perform client assessments, provide education to patients and/or their families, and monitor patients’ condition
Qualifications:
At least one year of related experience, preferably in Home Health.
Comfortable utilizing an EMR; experience with HCHB a plus.
Experience with OASIS and completing Start of Care visits.
Additional Information:
Base Pay, depending on qualifications and subject to negotiation: $60.00 - $65.00 per point.
$10,000 Sign on Bonus for Full Time
