Your Role: Home Health Physical Therapist Assistant
As a Physical Therapist Assistant in a home health setting, you’ll work one‑on‑one with patients to restore mobility, reduce pain, and prevent falls, empowering them to live safely and independently at home.
Key Responsibilities
Provide in‑home therapeutic care under the direction and supervision of a licensed Physical Therapist, following the established Plan of Care.
Implement therapeutic exercises, balance training, and mobility interventions to help patients improve function and achieve their rehabilitation goals.
Monitor and document patient progress, reporting changes in condition, functional status, or response to treatment to the supervising Physical Therapist and clinical team.
Educate patients and caregivers on safe mobility techniques, home exercise programs, use of assistive devices, and fall‑prevention strategies.
Document visits using WellSky Kinnser EMR.
Coordinate care with physicians, RNs, case managers, and the interdisciplinary team.
What We’re Looking For
California‑licensed Physical Therapist Assistant (PTA)
CPR certification (AHA‑approved)
At least 1 year of clinical experience (home health experience preferred)
Organized, compassionate, and reliable
Valid driver’s license, auto insurance, and reliable transportation
Strong communication skills and clinical decision‑making
