
Registered Nurse (RN) Rapid Response Team

Overview
Delivers patient care through assessment of patient, prioritization and implementation of interventions to ensure optimal patient outcomes, documentation of interventions and outcomes, communication with other team members and disciplines, and safe delivery of age appropriate and culturally aware patient care. Provides clinical expertise and intervention during rapid response to codes including code blue, code brain and rapid responses. Serves as a resource/support to staff, physicians, patients, families, and other departments. Is responsible for Rapid Response calls, follow-up visits, Rapid Response data collection, entry and analysis, development of Rapid Response practice guidelines, evaluation of patient codes and informal education of staff.

What You Will Do
Acts as a clinical expert and resource to maintain and improve patient care. Provides clinical expertise and intervention during rapid responses to codes including code blue, brain attack and rapid responses. Provides clinical assistance to staff members and demonstrates the use of organizational safety tools and education/mentorship of staff working directly with clinical staff and unit directors to promptly identify patients at risk for clinical deterioration and make recommendations aimed at improving overall clinical outcomes.
Use electronic medical records (EMR) data analytics for sepsis trending and proactively work with the clinical team for early identification of sepsis or rapid response codes. Evaluation and response to Sepsis Alerts/trending.
Proactively rounds on patients with a 6-hour sepsis protocol to help with core measures and standards of care. Rounds on patients.
Facilitates a collaborative environment that maximizes interdepartmental cooperation for positive results. Collaboration with both nursing and provider teams. Rounds on the unit educating nursing on early identification of sepsis and when to call a rapid response.
Qualifications
Education
Bachelors Degree, Nursing (Preferred) or
Associates Degree, Nursing (Preferred) or
Program Graduate, Nursing (Required) and
High School Diploma or GED, Only Required if Nursing Diploma Graduate (Required)
Experience
2 years of RN Critical Care (ICU or ER) work experience, OR 3 total years of RN experience with at least one year in critical care as an RN (ICU or ER) (Required)
Licenses and Certifications
Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) Upon Hire(Required)
CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) Upon Hire(Required)
ACLS Provider - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) Upon Hire(Required)
