Responsibilities
Under the direction of the Assistant Director and Clinical Manager (LCSW) of Ambulatory Care Management, you will work collaboratively with the patient, Faculty Practice Group, community resources, other providers, managed care operations, the UCLA Health System and health plans to implement, coordinate, and monitor options and services to meet at-risk patients’ healthcare needs through communication and coordination with providers, Comprehensive Care Coordinators, and staff at the practice site and in the appropriate UCLA Health System units to promote quality, cost-effective outcomes and appropriate level and mix of care. You will provide direct social work services, including assessment, intervention, and recommendations to individual patients and their families. You will participate in interdisciplinary activities, comply with departmental standards regarding attendance, documentation, continuous quality improvement, statistics, departmental policies and procedures, and the Code of Ethics. You will also keep current with professional social work health care knowledge base and pursue further enhancement of job-related knowledge.
