Evaluate residents and develop individualized treatment plans
Provide therapeutic interventions to improve mobility, strength, and function
Collaborate with interdisciplinary team members to ensure optimal patient outcomes
Maintain accurate and timely documentation in compliance with regulationsEducate residents and families on treatment plans and progress
Ensure compliance with all state and federal guidelines
Qualifications
Valid Physical Therapist (PT) license in the state of New York
Experience in a skilled nursing or long‑term care setting preferred
Strong clinical and communication skills
Ability to work effectively as part of a team
