Role Summary
The Health Guide role is responsible for assisting patients as they obtain care at the community-based clinic serving people experiencing homelessness. This position is responsible for the initial assessment and determination of the appropriate level of service needed to address medically and socially challenging issues, health needs, and barriers for patients. The Health Guide will provide education around resources and programs, and assist patients with navigating the system and with applications as needed.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
Education: Bachelor's degree in community health, social work/social services, psychology, or closely related field preferred; two years directly relevant experience in a healthcare or social services setting, working with vulnerable populations may be accepted in lieu of a Bachelor's degree.
License/Certifications: N/A
Experience: Relevant experience providing advanced knowledge of community outreach resources, Federal/State programs and skills in assessing client needs around their health and health care required.
Additional Skills/Requirements Required: Microsoft Word, Excel, and use of electronic consumer record software.
