Role Description
This on-site, part-time role, based in Geneva, IL, involves coordinating and managing care plans for seniors in need of assistance. As a Care Coordinator, you will oversee client schedules, ensure continuity of care, collaborate with caregivers and family members, and provide exceptional customer service. You will play a vital role in maintaining the quality of care and ensuring the well-being and satisfaction of our clients and their families.
Qualifications
Competency in Care Coordination and Case Management to manage client care plans efficiently.
Excellent Communication skills for engaging with clients, caregivers, and families effectively.
Experience in Home Care services to understand and cater to the needs of seniors.
Knowledge of Wellsky home health care system.
Strong Customer Service skills to ensure client satisfaction and resolve concerns promptly.
Proficiency in organizational and time-management skills to handle multiple responsibilities effectively.
Knowledge of senior care best practices and ability to operate in a compassionate and professional manner.
Previous experience in a similar role or in healthcare administration is a plus.
