to join our growing team.
This is a full-time hospice nursing position focused on
admissions and patient visits in the field , with a supportive interdisciplinary team, reasonable expectations, and leadership that understands hospice nursing.
Job Summary
The
Hospice Visit/Admissions Nurse (RN)
is responsible for completing hospice admissions and providing skilled nursing visits to patients in their homes. This role plays a key part in ensuring a smooth transition into hospice services while delivering compassionate, high-quality end-of-life care.
You will collaborate with physicians and the interdisciplinary team (IDT) to support patient comfort, dignity, and care continuity.
This role is ideal for an RN with hospice or home health experience who values autonomy, strong patient connections, and a balanced schedule.
Key Responsibilities
Perform hospice admissions, including initial assessments and start‑of‑care documentation
Provide routine and PRN skilled nursing visits as needed
Educate patients and families on hospice philosophy, services, and care plans
Collaborate with hospice physicians and interdisciplinary team (IDT), including aides, social workers, and chaplains
Support symptom management and end‑of‑life care needs
Ensure accurate, timely, and compliant documentation
Participate in team meetings and care coordination activities
Qualifications
Active Registered Nurse (RN) license – New Jersey
Hospice or home health nursing experience preferred
Strong clinical, assessment, and communication skills
Ability to work independently in a field‑based setting
Valid driver's license, reliable vehicle, and auto insurance
