Responsibilities
Conduct comprehensive nursing assessments of patients' physical, emotional, and psychosocial needs.
Develop, implement, and evaluate individualized care plans in coordination with the interdisciplinary team (including physicians, therapists, social workers, and aides).
Provide direct skilled nursing care (wound care, medication management, IV therapy, symptom management including pain and comfort in hospice) and patient/caregiver education.
Offer education and emotional support to patients and their families/caregivers based on individual needs.
Supervise LPNs/LVNs, home health aides, and other support staff.
Participate in on‑call rotations, admissions, or urgent response as needed.
Coordinate care activities, including visit scheduling and documentation to ensure compliance with regulations.
Monitor patient progress, respond to changes in condition, and adjust care plans accordingly.
Maintain accurate clinical records and ensure care aligns with agency policies, professional standards, and regulatory requirements.
Qualifications
Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
One (1) year experience as an RN.
Licensed to practice as a registered nurse in the state of agency operation.
Must possess and maintain valid CPR certification while employed in a clinical role.
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order; able to travel to all business locations.
