Home Helpers Home Care is currently looking for a PRN RN to complete initial client assessments and re-evaluations of client needs. Ability to initiate care plans and necessary revisions.
Duties
Performs supervisory visits of home health aides.
Provides services in accordance with the plan of care.
Makes the initial evaluation visit and regularly reevaluates the client’s needs.
Initiates the plan of care and necessary revisions.
Provides documentation of client assessments, plan of care, supervisory visits, caregiver and client education and training.
Informs personnel of changes in the condition and needs of the client.
Understands and adheres to established policies and procedures.
Observes signs and symptoms and reports reactions to non‑medical treatments, including changes in the patient’s physical or emotional condition.
Teaches and counsels the patient and family/significant others regarding their service needs and other related problems of the patient at home.
Maintains accurate and complete records of observations, non‑medical treatments and care of patient.
Participates in medical record audit as assigned.
