Coordinate care with physicians, therapists, and the Director of Nursing
Document patient visits in compliance with home health regulations
Qualifications – Home Health RN Jobs
Active California Registered Nurse (RN) license (required)
Minimum 1 year RN experience (home health experience preferred)
Experience in rehab, geriatric care, or community health is a plus
Strong clinical, communication, and organizational skills
Valid driver’s license and reliable transportation
Ability to travel throughout Los Angeles County – Torrance, Compton, Gardena
Preferred Qualifications
Home Health RN experience
Public Health Nurse (PHN) certification
Experience with mobile therapy or home-based care models
