Symbia is looking for a skilled and passionate Full‑time Physical Therapist to join our employee‑owned team at DeKalb County Nursing Home in DeKalb, IL.
$2,000 Sign‑On Bonus

This position can be part‑time (20 hours per week, flexible schedule) or full‑time (30 hours per week, flexible schedule).
Responsibilities
Establish goals and design personalized treatment plans to meet patient needs.
Supervise Physical Therapist Assistants to ensure effective care.
Maintain productivity standards and complete required documentation.
Collaborate as a key member of the interdisciplinary team, making a meaningful impact in the lives of patients.
Help patients achieve critical health outcomes and contribute to the overall well‑being of the community served.
Qualifications
Graduate of an accredited Physical Therapy program.
Valid state license or pending license (for newer graduates).
Experience in acute care, long‑term care, or rehabilitation is a plus, but not required.
