Palliative Care Nurse Practitioner

Position Summary
The Palliative Care Nurse Practitioner is responsible for modeling Compassus values and promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. They provide palliative care for patients facing chronic, complex, or life‑threatening conditions at various locations, performing assessments, education, and symptom management to maintain patient comfort and coping. The role includes collaboration with PCP, specialists, and the assigned Compassus Medical Director, with flexibility to meet agency needs.

Responsibilities

Perform routine and urgent assessments.

Availability Monday – Friday, 8a‑5p local time.

Complete a minimum of 100 visits per month, adjusted for PTO and part‑time status.

Carry a minimum patient census load of 80 patients, may increase depending on market needs.

Triage consults by patient acuity.

Provide bedside palliative care with patient/family education.

Subject to after‑hours and weekend on‑call requirements as needed.

Order, perform, and interpret laboratory and radiology tests within scope of practice.

Prescribe medications, including controlled substances, within delegated scope.

Order treatments and durable medical equipment as indicated.

Perform other therapeutic measures as indicated.

Consult with palliative care physician or designees as needed; inform primary physician of services and collaborate with other physicians.

Assist in all facets of care coordination for palliative care referrals.

Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement.

Comply with applicable laws and regulations regarding collaborative agreements.

Complete accurate billing and coding activities for all patient encounters per CMS guidelines.

Provide training, ongoing education, and onboarding support for staff.

Define goals for professional growth and participate actively in professional activities and organizations.

Engage in active and frequent self‑care activities for personal and professional growth and longevity.

Adhere to the confidentiality practices regarding patients, families, staff, and the Organization.

Assist patients and families in identifying goals of treatment and ongoing plans of care at every visit; communicate these goals among the interdisciplinary team (IDT).

Collaborate and communicate with Medical Director, attending physician, hospital staff, and IDT; participate in monthly IDT meetings with market team; assist in identifying intervention needs of other IDT members; effectively communicate patient and family needs; complete and submit required clinical documentation within 24 hours of visit completion.

Educate patient and caregiver regarding care of patient, disease process, goal setting, symptom control, treatment options, prognosis, and advance care planning.

Supervise nurse trainees, including graduate and advanced practice nurses in field placements.

Participate in supervision of other trainees, including medical students, residents, fellows, and social work and psychology trainees.

Require regular regional travel, involving driving within a 60‑mile radius.

Perform other duties as assigned.

Education and Experience

Master’s degree in Nursing required.

Minimum of five (5) years nursing experience; clinical nursing care in oncology, hospice, geriatrics, or intensive care preferred.

Skills

Mathematical skills: add, subtract, multiply, divide in all units; compute rate, ratio, percentage.

Language skills: read, analyze, interpret business periodicals, professional journals, technical procedures, or governmental regulations; write reports, business correspondence, procedure manuals; present information and respond to questions; strong written and verbal communication.

Other skills: understand, read, write, speak English; articulate hospice philosophy; strong organizational and time‑management skills; work flexible schedule including nights and weekends; travel; maintain confidentiality of patient and company information.

Certifications, Licenses, and Registrations

Active and unencumbered CNS or Nurse Practitioner license in state of employment.

Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric.

Active NPI number.

Maintain active DEA certification if state allows prescription of controlled substances by APPs.

Board Certification in Hospice and Palliative Care preferred but not required.

Physical Demands and Work Environment
This role requires regular standing, walking, and manual dexterity. Ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. Exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
