Responsibilities
LPN Referral Management PRN Nights to work 12‑hour shifts. The LPN serves to assess and coordinate the care of patients referred for mental health services.
Review assessment referrals received and collaborate with RN to assess patient needs.
Assist with coordination of patient care.
Maintain patient records, updating databases, and ensuring accurate documentation of patient information.

Qualifications
Education : Nursing degree, preferred Bachelor’s or Master’s.
Licensure/Certification : Licensed LPN in current State or compact license; current CPR if required by customer hospital.
Experience : Preferred 1‑3 years related work experience in behavioral health.
