Job Description
Sun River Health provides comprehensive primary, preventative, and behavioral health services to all who seek care, regardless of insurance status or ability to pay. We are a Federally Qualified, Non‑Profit Health Center serving communities in New York City, Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia, and Westchester. Sun River Health is currently seeking a full‑time Registered Nurse / MAT Care Manager to join our busy team at the Riverhead site.

Position Summary
The Registered Nurse / MAT Care Manager is a key member of our multidisciplinary team, using the nursing process to assess, plan, implement, and evaluate patient care and resource use. The role monitors quality of care, ensures infection control and risk management, serves as patient advocate, provides care and crisis management, and understands harm reduction approaches. Responsibilities include opioid overdose prevention education, staff and patient training, and participation in multidisciplinary case conferencing.

Pay Range
$45.00 - $52.11 per hour

Sign‑On Bonus
$2,700 for eligible external applicants (terms and conditions apply).

Essential Duties and Responsibilities

Act as the primary point of contact and champion for the site location.

Complete MAT intakes by phone or in person.

Develop and document MAT patient care plans and recovery support plans.

Document all encounters and coordinate care through necessary consent forms.

Prepare discharge paperwork in conjunction with the MAT provider.

Triage presenting issues and provide basic nursing skills for every visit, including pre‑visit planning and template management.

Implement opioid overdose prevention protocols (I‑STOP, PNP, prior authorizations, lab review, urine screening).

Provide case management to MAT patients, manage referrals, and present at weekly case conferences.

Monitor the MAT panel using internal spreadsheets for case conference, discharge, and patient master lists.

Provide opioid overdose prevention education and training to patients and staff.

Participate in MAT program initiatives and other duties as assigned.

Essential Job Qualifications

Minimum Education: Bachelor’s Degree.

Minimum Experience: 1-2 years in an acute care setting.

Licensure and Certification:

New York State nursing license.

BLS certification.

CPR certification.
