LVN Charge Nurse (Night Shift 6PM-6AM) – $3000 sign‑on bonus

Job Summary
Provide quality nursing care to residents at The William R. Courtney Texas State Veterans Home. Coordinate total nursing care, enforce STGi policies, state and federal regulations, and support resident well‑being.

Responsibilities

Follow established standards of nursing practice, implementing STGi programs, policies and procedures.

Supervise and evaluate direct resident care, initiating corrective action as necessary.

Obtain and record reports from relieving nurses; provide competent hand‑over to incoming nurse.

Arrange staffing to meet residents' needs.

Conduct daily resident rounds, report problems, and initiate corrective action.

Identify safety hazards and launch corrective action immediately.

Respond to resident emergencies per physician orders; administer emergency care when physician absent.

Provide supervision to Nursing Assistants delivering direct resident care.

Attend staff meetings, interdisciplinary team meetings, rehabilitation meetings, in‑service classes, and committees as assigned.

Review Glucometer Control Log and ensure proper monitoring of blood glucose levels.

Assign nursing duties based on staffing qualifications and resident characteristics.

Advise Nursing Assistants and recommend disciplinary action to the Director of Nursing Service when needed.

Maintain community processes related to pain assessment, documentation, medication and treatment delivery.

Administer and document direct resident care, medications and treatments per physician orders; record care accurately.

Teach licensed nurses and Nursing Assistants proper nursing care procedures.

Document resident assessments per licensing; report unsatisfactory findings to physician and supervisor.

Observe the 5 Rights of medication/treatment administration.

Report changes in resident conditions promptly to physician, Director of Nursing Service, and responsible parties.

Communicate resident admission/transfer procedures with other departments and rehabilitation.

Perform basic nursing skills competently as authorized by licensing.

Coordinate resident call response and supervise response to aids.

Listen to resident, family, or physician concerns and initiate corrective action or notify Nursing Supervisor.

Count narcotics, check emergency kit, and exchange keys each shift.

Verify meal trays and ensure appropriate diet; assist residents as needed.

Order, receive, and store medications properly.

Execute community process for narcotic sign‑out and documentation each shift.

Notify physician of changes in resident conditions, medication errors, abnormal labs or X‑rays.

Document daily notes on unstable residents, Medicare residents, and recently admitted residents.

Ensure safety of residents and team members under your supervision.

Observe infection control procedures.

Perform all duties of a certified nursing assistant when required.

Required Skills & Experience

Must hold a current and valid LVN nursing license in Texas or a valid LVN/LPN license issued by a Compact Party State.

Current Basic Life Support (BLS)/Cardiopulmonary Resuscitation (CPR) certification required before work, preferably from the American Heart Association (AHA). Ability to obtain certification within 30 days is acceptable.

Working Conditions / Physical Requirements
The role involves standing, walking, twisting, reaching, bending/stooping, pushing and pulling typical weights of 5–30 lb (approximately); lifting 5–100 lb (approximately). Requires repetitive finger movements, hearing and speaking on the phone and in person. Must manage stress created by residents, multiple tasks, noise, and interruptions while maintaining a pleasant attitude.
