Service Facilitator (SF) Case Manager

At Moms In Motion, our Service Facilitators are everyday heroes. You’ll spend your days connecting with families, helping them navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT), and making sure those you serve can live safe, healthy, and independent lives.
We’re looking for compassionate, organized, and community-focused individuals who love the idea of making a difference.
What You’ll Do
Travel locally within Virginia to visit clients in their homes.
Serve as the go-to guide for families navigating Medicaid Waiver programs.
Create and document plans of care and assessments that make a real difference.
Troubleshoot challenges such as service authorizations and timesheets.
Build lasting relationships with families based on respect, patience, and trust.
Work remotely from your laptop or tablet.
Maintain accurate documentation and stay compliant with program requirements.
What We’re Looking For
At least 2 years of experience supporting individuals with disabilities or the elderly.
No degree required; we value heart and experience over formal education.
Tech-savvy: comfortable with email, documents, spreadsheets, portal navigation, and video calls.
Valid driver’s license and reliable vehicle (but no transportation of clients).
Excellent written and spoken communication skills.
Ability to pass a background check and provide two professional references.
