As the
RN Case Manager
for Maternity and/or NICU, you will have a panel of members with high‑risk pregnancies and/or NICU babies with complex medical or behavioral needs. You will provide support and address barriers to care and social determinants of health with the goal to improve health outcomes.
To promote healthy pregnancy, delivery, and postpartum outcomes for both mother and baby, it is essential to provide evidence‑based, trimester‑specific and postpartum education. This includes assistance in developing a birth plan, accessing value‑added benefits, and understanding covered services, including Medicaid postpartum coverage. Additionally, support should be given for accessing contraceptive care based on family planning needs, conducting mental health and substance use disorder screenings with referrals to treatment when necessary, and coordinating with obstetric and other providers for appointment assistance, communication, and advocacy.
This is a field‑based role with a home‑based office.

or surrounding areas, you will have the flexibility to work in both the field and telecommute as you take on some tough challenges.
Primary Responsibilities
Provide evidence‑based trimester‑specific and postpartum education to promote a healthy pregnancy, delivery, and postpartum outcomes for mother and baby.
Collaborate with your team, the member, their family/support group, and healthcare providers.
Understand insurance products, benefits, coverage limitations, and governmental regulations as it applies to the health plan.
Help member set goals and develop a care plan to achieve those goals.
Adhere to detailed, specific documentation requirements in the member's health record.
Proactively engage the member to manage their own health and healthcare.
Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services, and mental health and substance use treatment.
Perform outreach to providers, hospitals, members, and clinics.
Attend community events.
Be able and willing to travel within the assigned region per the business needs.
Utilize solid skill sets of managing multiple tasks at a time, being self‑motivated, driven toward quality results, managing time well, being very detailed oriented and organized, work well in a team and on your own, and ability to manage multiple deadlines.
Perform all other duties assigned.

Required Qualifications
Current, unrestricted independent licensure as a Registered Nurse in Kansas.
Must have resided within the local community for 2+ years.
2+ years of experience working within the maternal and child health population.
Intermediate level of proficiency with MS Office, including Word, Excel, and Outlook.
Access to a designated quiet workspace in your home (separated from non‑workspace areas) with the ability to secure protected health information (PHI).
Must be able to comply with UHG Telecommuting Policy and have a designated workspace inside the home with access to high‑speed internet availability.
Ability to travel up to 75% of time in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices.
Access to reliable transportation & valid U.S. driver’s license.
Preferred Qualifications
Case management experience.
Experience with electronic charting.
Background in managed care.
Knowledge of the culture and values of community.
Bilingual (Spanish).

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401(k) contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $28.94 to $51.83 per hour based on full‑time employment. We comply with all minimum wage laws as applicable.
