We’re looking for a compassionate, skilled Home Health Nurse who values connection just as much as clinical excellence. In this role, you won’t just treat conditions—you’ll build relationships, restore independence, and make a real difference in people’s daily lives.

As part of our team, you’ll provide one‑on‑one care to patients with a variety of needs, from post‑surgical recovery to chronic disease management. You’ll assess, plan, and deliver skilled nursing care while working closely with physicians, patients, and families to ensure the best outcomes. Every home you enter brings a new story, a new challenge, and a chance to leave someone better than you found them.

Full‑time and part‑time hours available.

Reports To
Clinical Manager / Alternate Clinical Manager / RN Case Manager

Position Summary
Provides skilled services to restore and promote patient’s health by completing the nursing process. Assures quality of care by measuring health outcomes against patient care goals. Maintains a safe and clean working environment in compliance with procedures, rules, and regulations.

Essential Functions / Areas of Accountability

Assess and chart observations of the patient’s condition at each visit.

Complete evaluation tasks, including reviewing medication and vital signs.

Administer physician‑prescribed medication.

Provide education to patients and families on proper home healthcare procedures and strategies.

Coordinate with physicians and other individuals in the patient’s plan of care.

Communicate effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.

Qualifications

Registered Nurse (RN) or Licensed Practical Nurse (LPN) with current state credentials.

Excellent interpersonal skills both written and verbal, time‑management, and Microsoft Office skills.

Clinical skills and detail oriented, organized.

Experience in or passion for home health or community‑based care.

Compassionate, dependable, and confident working independently.
