Join a 13‑week travel assignment in Tompkinsville, KY, with 8‑hour day shifts in a skilled nursing facility (SNF). Your responsibilities include assessing patient needs, developing individualized care plans, conducting therapy sessions, and collaborating with a multidisciplinary team to ensure comprehensive patient care. You will also educate patients and families on strategies to improve health outcomes and maintain accurate records of patient progress and treatments.
Required qualifications include a current professional license in North Carolina and experience in an SNF setting. Strong communication and teamwork skills are essential, and familiarity with electronic medical record (EMR) systems is recommended.
