About the Role
As an
Admissions Registered Nurse – Home Health
at Corus Health, you are often the first clinical connection a patient has with our team — setting the tone for everything that follows. In this role, you’ll complete in-home assessments, guide patients and families through the start of care, and build the clinical foundation for safe, effective treatment at home.
This is a fast-paced, highly independent role designed for experienced home health nurses who are confident in their judgment, organized in their approach, and calm under pressure. You’ll be supported by a leadership team that values clarity, trust, and collaboration — while giving you the autonomy to do your best work in the field.

Key Responsibilities
Conduct initial in-home nursing assessments to determine patient eligibility and care needs
Develop and initiate individualized plans of care in collaboration with physicians and the interdisciplinary team
Complete OASIS documentation and all admission paperwork accurately and within required timeframes
Educate patients and caregivers on the home health process, care plans, and expectations
Coordinate seamless transitions into home health services, ensuring continuity and clarity of care
Communicate effectively with physicians, care coordinators, and internal team members
Requirements
Active RN license in the state of practice; graduate of an accredited nursing program
Required:
Prior home health experience, including admissions and OASIS documentation
Strong attention to detail and ability to stay highly organized in a fast-paced environment
Calm, steady clinical presence — especially during high-volume or time-sensitive situations
Ability to work independently with confidence and sound clinical judgment
Excellent communication and interpersonal skills with patients, families, and care teams
