Overview
New Role: Patient Care Navigator (MA Preferred)
Rate: $25–$27 per hour
Helping patients feel supported, informed, and connected every step of the way.
We’re looking for a Patient Care Navigator who genuinely enjoys helping people and guiding them through their healthcare journey.
In this role, you’ll be the bridge between our patients and the programs we offer, making sure they feel confident, educated, and supported both before and after their visits. You’ll introduce patients to services like Remote Patient Monitoring (RPM), Chronic Care Management (CCM), Advanced Primary Care Management (APCM), Home Care, and Adult Day Care, helping them access the care that fits their needs best.
This is a hands-on, patient-facing role for someone who is kind, approachable, and loves educating others.
Responsibilities
Guide & Support Patients – Help patients navigate their care options in a simple, clear, and supportive way
Connect patients to programs like RPM, CCM, APCM, Home Care, and Day Care services
Be a go-to person patients can trust for answers and direction
Educate & Empower – Explain our programs in a way that’s easy to understand
Show patients how to use devices (like RPM equipment) when needed
Make sure patients feel confident managing their care
Prepare & Follow Up – Review patient schedules ahead of time to identify who may benefit from certain programs
Check in with patients before or after visits to guide next steps
Ensure no patient “falls through the cracks”
Coordinate Care – Help patients schedule appointments and services
Assist with Primary Care Provider (PCP) changes if needed
Work closely with providers and team members to ensure smooth care transitions
Create a Great Experience – Build real relationships with patients
Bring positive energy and compassion into every interaction
Make every patient feel seen, heard, and cared for
Qualifications
Medical Assistant (MA) background preferred
Experience in a patient-facing healthcare role
Strong communication skills, you can explain things simply and clearly
Comfortable teaching patients and demonstrating devices
Organized, reliable, and proactive
