FOX Rehabilitation is seeking a motivated and compassionate Physical Therapist to join our clinician‑owned and operated private practice specializing in geriatric rehabilitation. As a Physical Therapist with FOX Rehabilitation, you will provide evidence‑based, patient‑centered care to older adults, helping them maintain independence, improve functional mobility, and enhance overall quality of life. You will work primarily with a geriatric population across a variety of settings—such as patients’ homes, senior living communities, and other on‑site locations—delivering individualized treatment plans that focus on meaningful, functional outcomes.
In this role, you will perform comprehensive evaluations, develop and implement personalized plans of care, and deliver skilled physical therapy interventions including therapeutic exercise, balance and gait training, fall prevention strategies, pain management, and functional mobility training. You will regularly reassess progress, modify treatment plans as needed, and utilize standardized outcome measures to ensure clinical effectiveness and quality. Accurate, timely documentation in the electronic medical record is an essential component of the position.
You will collaborate closely with an interdisciplinary team of healthcare professionals, caregivers, and family members to coordinate care and advocate for your patients’ needs. Education and coaching of patients and their support systems on home exercise programs, safe mobility techniques, and strategies to maximize independence are key aspects of your daily work. You will also conduct home and environmental safety assessments, recommending adaptive equipment or modifications to support safe, functional living.
