Minimum Qualifications

This position requires a Bachelor’s Degree in the field of Human Services with two years of experience providing direct services to youth and families with complex needs. Strong verbal and written communication skills are necessary. Bilingual Spanish preferred. Must have the ability to gain a working knowledge of team facilitator practices, procedures and Wraparound principles, and also be able to accurately and efficiently complete crisis plans, functional assessments, individual Wrap plans and progress notes. Must have the ability to meet Wraparound Facilitator certificate requirements within one year of employment, and provide facilitation of initial and follow-up team meetings. Must demonstrate a commitment to strengths based practice. Proficiency in interpersonal social and communication skills, as well as organization and planning are necessary. Must have an ability to maintain social boundaries with youth and families, while exercising good judgment, including the ability to assess and respond to dynamics of family relationships. An ability to apply crisis planning and functional assessment skills is also important. Must have an ability to maintain professional behavior at all times, and to work as a member of the Mission Possible team. Must pass background investigations; CBI, FBI, and Central Registry background checks.
Position Description

Wraparound Facilitation is a process of The Wraparound Facilitator is responsible for the implementation of the assigned client’s Individual Wraparound Plan and providing facilitation and support to designated youth and families. Ensuring team formation and facilitation is a major component of the job duties.
Specific Responsibilities

Prepare youth, families and other team members for the Wraparound process.
Strictly adhere to the principles, phases, and activities of High Fidelity Wraparound.
Complete Strengths, Needs, Culture Discoveries.
Assist the family in identifying their vision.
Maintain regular meetings and home visits with clients to ensure compatibility and progress with the Wraparound plan.
Write updated wraparound plans to reflect the SNCD of the youth and families.
Ensure that the youth and families needs are being addressed while the family is progressing through the Wraparound process.
Actively participate in the Wraparound coaching process to include individual and group coaching sessions.
Provide and keep updated a weekly calendar of scheduled appointments and office hours. These schedules must account for a full allocation of hours to be worked.
Support the engagement of families with SUD in their own treatment and recovery services
Gather GPRA data as required for grant compliance
Conduct appropriate screening, data collection, and fidelity tools
Support, encourage and provide feedback to the youth, young adult, and families.
Attend Wraparound training and other assigned trainings and workshops that would help assist and educate.
Facilitate and coordinating access to services described in the Wraparound Plan, including therapeutic care, education, socialization, recreation, remedial therapy and natural supports.
Provide on-call and crisis-intervention services and functional assessments as needed.
Perform other duties as assigned by the Wraparound coach and/or the Executive Director.
