Clinical Social Worker Essential Job Duties
The Clinical Social Worker is responsible for performing assessments appropriate to functional status, diagnosis, care setting, age, development level, and cultural and religious orientation in a timely manner. The candidate will also identify and prioritize the need for further assessment based on the patient’s clinical presentation/diagnosis, care setting, desire for care and responses to any previous care. The Social Worker collaborates with Case Managers on discharge planning.
Qualifications
Two (2) years social service/discharge planning/case management experience, preferably in an acute care hospital
Completion of an accredited Social Work Program
MSW or LCSW required
