Summary
Social Workers are part of the Case Management Team with the inpatient Care Coordination Department. Referrals for social work intervention require evaluation of needs to be met, understanding specific patient‑centered outcomes, and accountability of action within a case‑management team structure. Medical Social Workers assist with patient discharges from the hospital, coordinate care of patients with complex needs that require multiple services over time, and handle crisis intervention. The Medical Social Worker I intervenes with patients who have complex discharge and psychosocial needs, assists with eligibility determination for social programs and funding sources, and qualifies patients for community assistance from various special funds and agencies. In addition, offers crisis intervention to patients and families and coordinates a discharge plan of care.

Responsibilities
Assess patients and families psychosocial risk factors based on referral, consultation or case finding
Provide initial screening in the ED for crisis intervention and overcapacity situations
Intervene with patients and families regarding emotional, social, and financial consequences of illness; mobilize resources; provide crisis management
Support and counsel patients/families anticipating or experiencing adjustment issues, catastrophic diagnosis, living‑situation changes, and bereavement
Provide interventions for child abuse
eglect, domestic violence, elder abuse, institutional abuse, and sexual assault
Advocate for patient and family empowerment and independence
Participate in discharge planning to ensure timely discharge and appropriate post‑acute linkage
Collaborate with case manager and multidisciplinary team on discharge planning
Educate patient/family and physician regarding post‑acute options and choice issues
Participate in complex rounds, discharge‑planning huddles, unit daily whiteboard rounds
Lead assistance in obtaining financial and other resources (Medicaid, SSDI, County Indigent Funds, UNM Cares, etc.)
Assist with end‑of‑life matters when indicated
Meet CMS Condition of Participation requirements and applicable regulations
Serve as lead in obtaining legal guardianship and competency determinations; document accordingly
Facilitate and lead patient/family and provider care conferences as needed
Ensure safe care by adhering to policies, procedures and standards, within budgetary specifications
Maintain and update community services resources, advice, and referral information
Promote professional growth and meet mandatory/continuing education and skill‑competency requirements; serve as preceptor and mentor
Document assessments, interventions and discharge planning in the medical record according to policies
Other duties as assigned
Qualifications
Master’s degree in Social Work required
Current license from New Mexico: Licensed Independent Social Worker (LISW), Licensed Clinical Social Worker (LCSW), or Licensed Medical Social Worker (LMSW)
Experience:
Minimum 10 years hospital social work or discharge planning (Social Worker III level)
Minimum 5 years hospital social work or discharge planning (Social Worker II level)
Minimum 6 months hospital social work or discharge planning (Social Worker I level)
National Case Management certification preferred
Licensed Medical Social Work License (LMSW) preferred
