The Advanced Practice Nurse (APN) III Certified Nurse Practitioner (NP) practices in Home Based Primary Care (HBPC) in the Rehabilitation and Extended Care (REC) Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
The NP provides advanced clinical practice - consultation - and management
Duties include: Facilitates collaboration of the interdisciplinary Home Based Primary Care (HBPC) staff members and those located throughout the medical center - promoting a team approach for the coordination of care
Completes detailed history and physical exam - ordering emergent and screening diagnostic tests - review and interpretation of diagnostic test results Evaluation and management of symptoms and functional status - Care coordination/recommendations - education - and counseling about goals of care and advanced care planning
Facilitates interdisciplinary patient care conferences - assisting in the development of appropriate individualized and goal-oriented plans of care
Initiate consults to other services - including specialty departments as appropriate
Document assessment process accurately - clearly - and concisely when developing initial and ongoing plans for patient care
Consistently makes sound judgments based on accurate and thorough assessments
Consults and collaborates with other health team members
Other duties as assigned within privileges and licensure
VA offers a comprehensive total rewards package: VA Nurse Total Rewards Pay: Competitive salary - regular salary increases - potential for performance awards Paid Time Off: 50 days of paid time off per year (26 days of annual leave - 13 days of sick leave - 11 paid Federal holidays per year) Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Licensure: 1 full and unrestricted license from any US State or territory Work Schedule: 7:30am - 4:00pm.
