Duties

The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers

This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest

As an employee operating in a Whole Health System of care - you will operate in a model with three core elements - seeking to create a personalized health plan for each Veteran

This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community

This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being

The Advanced Practice Nurse (APN) III Certified Nurse Practitioner (NP) practices in Home Based Primary Care (HBPC) in the Rehabilitation and Extended Care (REC) Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission

The NP provides advanced clinical practice - consultation - and management

Duties include: Facilitates collaboration of the interdisciplinary Home Based Primary Care (HBPC) staff members and those located throughout the medical center - promoting a team approach for the coordination of care

Completes detailed history and physical exam - ordering emergent and screening diagnostic tests - review and interpretation of diagnostic test results Evaluation and management of symptoms and functional status - Care coordination/recommendations - education - and counseling about goals of care and advanced care planning

Facilitates interdisciplinary patient care conferences - assisting in the development of appropriate individualized and goal-oriented plans of care

Initiate consults to other services - including specialty departments as appropriate

Document assessment process accurately - clearly - and concisely when developing initial and ongoing plans for patient care

Consistently makes sound judgments based on accurate and thorough assessments

Consults and collaborates with other health team members

Other duties as assigned within privileges and licensure
