Job Summary
The Community Outreach Specialist supports Fountain of Life initiatives to the Burmese‑speaking community. The role serves as a health navigator, community health worker, and social worker, helping connect individuals and families in need to health, wellness, aging, social, housing and legal services. When individuals whose primary language is Burmese reach out to Fountain of Life, the specialist will be their first point of contact.
Job Responsibilities
Connect Burmese‑speaking individuals and families, especially those uninsured or underinsured, to health, aging, government, and community resources, including medical, dental, and behavioral health care to lead healthy lives.
Promote Fountain of Life social, recreational and mental health programs to Burmese‑speaking older adults.
Represent Fountain of Life to the Burmese‑speaking community of Greater Kansas City.
Help develop and maintain relationships between Fountain of Life and Burmese‑serving community organizations, churches, and groups.
Conduct intakes to determine health, wellness, aging, social, housing and legal needs.
Educate and connect individuals and families to community resources and services to meet their needs.
Make follow‑up phone calls and serve as point of contact for Burmese‑speaking individuals reached through health outreach events, providing continued care coordination and addressing barriers to accessing resources.
Maintain relationships with individuals and families and encourage participation in Fountain of Life’s programs and activities at its senior community center in Kansas City, Kansas.
Translate program flyers to Burmese and help distribute program information to the community.
Serve as Burmese interpreter at Fountain of Life’s programs and activities as needed.
Track program participant information according to Fountain of Life’s participation tracking procedures.
Required Qualifications
A sincere heart to build relationships among different Asian ethnic communities.
Ability to use computer applications, including Microsoft, Google Suite, and email.
Bilingual in Burmese and English.
Legal resident and eligible to work in the United States.
Reliable transportation / access to fully‑insured vehicle to use for job‑related duties.
Ideal Candidate Profile
Friendly and service‑oriented personality.
Flexibility and adaptability to fast‑paced and frequently changing environments.
Creative and innovative.
Attentive to detail, but non‑perfectionist.
Good steward of time and resources.

Compensation
$25 per hour for an 8‑month period (May 2026 to December 31, 2026).
