Job Description
Physical Therapist – Skilled (PT – Skilled)
You will evaluate patients, develop individualized treatment plans, and deliver skilled PT interventions focused on mobility, strength, balance, and functional independence. Responsibilities include monitoring patient progress, adjusting plans of care, documenting evaluations and treatments, and collaborating with an interdisciplinary rehabilitation team.
Start date: ASAP. Availability: 8 D shifts.
Required Qualifications
Graduation from an accredited PT or DPT program
Active Minnesota Physical Therapist license
1 reference from entire work history
Preferred Qualifications
Experience in a Skilled Nursing Facility (SNF)
Immediately available to start
Capable of split assignments between Redwood Falls and Sleepy Eye
