PRN Physical Therapist (PT) – Skilled Nursing Facilities (SNFs) Location: Near Winamac, Indiana (2 Facilities)

A growing healthcare organization is seeking a dedicated Physical Therapist (PT) to provide evaluation services across two skilled nursing facilities located near Winamac, IN. This role offers the opportunity to work with a collaborative interdisciplinary team while making a meaningful impact on residents’ quality of life.

Position Summary
The Physical Therapist will be responsible for completing evaluations, developing individualized treatment plans, and supporting rehabilitation goals for residents in a long-term care setting. This role focuses heavily on assessments and care planning, ensuring each patient receives the appropriate level of therapy services.

Key Responsibilities

Perform comprehensive physical therapy evaluations for new and existing residents

Develop and implement individualized treatment plans based on patient needs

Collaborate with therapy staff, nursing, and physicians to ensure coordinated care

Monitor patient progress and adjust plans as needed

Maintain accurate and timely documentation in accordance with regulatory standards

Provide guidance and support to therapy assistants as appropriate

Qualifications

Licensed Physical Therapist in the state of Indiana (or eligible)

Previous experience in skilled nursing or long-term care preferred

Strong clinical assessment and documentation skills

Excellent communication and teamwork abilities

Ability to manage time effectively across multiple locations
