Physical Therapist (PT) in Skilled Nursing Facility
Now staffing Physical Therapists for the Rehab Department in state‑of‑the‑art SNFs across NYC and surrounding areas. Flagstar Rehab PTs work with a diverse multi‑disciplinary team to perform evaluations, treatments, and patient/family education to improve physical and cognitive function and positively impact daily living.

Hourly Payrate:
W‑2: $46
1099: $57
Physical Therapist Job Duties
Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
Supervise PTAs daily.
Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, stroke, Parkinson’s, multiple sclerosis, cancer, orthopedic surgery, and other debilitating diagnoses.
Monitor and document patient progress using innovative EMR software.

Current NYS PT license and registration
CPR certification
