Summary
The VA Northeast Ohio Healthcare System is recruiting for a Licensed Practical Nurse who will work in Community Outpatient Services.

Basic Requirements
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Must be a graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing.
Education will be waived for current VHA employees who were converted to appointments under 38 U.S.C. 7401(3) or under 38 U.S.C. 7405. The waiver will not apply to persons appointed under these provisions after the conversions.
Must possess a full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, Commonwealth or District of Columbia.
May qualify under the Grandfathering Provision described in the VA Qualification Standard for this occupation (applicable to current VHA employees who meet the criteria).
Grade Determinations: GS‑07 – must possess at least one year of qualifying experience at the GS‑6 level or equivalent.
Must fully meet all performance requirements for the GS‑6 LPN/LVN or equivalent and resume must fully support this.
Experience examples include technical proficiency in initiating, performing, and completing assigned duties in providing care to variable patient populations; appropriately carrying out patient care based on patients' conditions; using judgment to select the appropriate order and sequence of procedures and treatments; accurately recognizing, reporting, and recording relevant patient information; and having work periodically reviewed by a registered nurse or physician for appropriateness.
Ability to observe, identify, and respond to patient needs for care, including medication, equipment‑assisted care, and family education; considering emotional, cultural, spiritual, socio‑economic, and age‑related factors.
Preparation and administration of prescribed medications (oral, topical, subcutaneous, intramuscular, and/or intravenous) and performing treatments according to established policies.
Monitoring patients for physical or emotional changes from medications or treatments, promptly documenting changes, and reporting deviations to RN or MD/DO.
Recognition of urgent or emergent patient care situations, seeking assistance of RN or MD/DO, and initiating appropriate emergency interventions as directed.
Knowledge and understanding of human behavior, patient motivations, reactions, and using this knowledge to work effectively with patients, families, and staff.
Establishing constructive relationships with patients and families to elicit feelings and attitudes and promote positive relationships, communication, and socialization skills.
Fostering an environment of respect for patient privacy and dignity in all aspects of care delivery.
Incorporating knowledge of customer service standards in all interactions with patients, families, and other customers.
Performing support duties for complex diagnostic tests and specialized procedures, including patient preparation, instrument handling, and monitoring conditions before, during, and after procedures.
Serving as a preceptor in orienting, educating, and training less experienced LPNs/LVNs or NAs/HTs related to support duties for complex procedures.
Preferred Experience: Two or more recent primary care or adult acute nursing care experiences.
Physical Requirements: Ability to climb stairs; lift, carry, pull, and push up to 25 pounds; frequent walking, standing, kneeling; repeated bending and squatting; drive a car for up to 2 hours.
Duties
Assist in planning, implementation, and evaluation of patient care with interdisciplinary teams.
Perform complex healthcare activities with minimal supervision.
Manage patient care problems and deviations using self‑direction in accordance with VA Nursing Practice and LPN licensure standards.
Maintain knowledge and skills to care for a wide range of patients, including those with chronic and acute illnesses.
Provide nursing care for adults with specific population needs, incorporating relevant physical, psychosocial, and cultural needs into the patient care plan.
Readily identify patient complications and act to resolve issues under general supervision.
Apply critical thinking skills to solve clinical and administrative issues.
Assist staff to identify Veteran medication and treatment needs and accurately communicate those needs to the team.
Collaborate with the PACT team and other interdisciplinary services in the development, implementation, and evaluation of patient care.
Provide indirect nursing care to patients with complex needs, including consult management, telephone screening, and nursing action accountability.
Demonstrate active listening and interviewing/screening skills during telephone calls to obtain pertinent information and utilize the computerized medical record as a resource.
Serve as a preceptor in orienting, educating, and training less experienced LPN staff for support duties related to complex procedures.
Determine clinic assignments, communicate with patients, manage consults, and review medical records.
Actively seek educational opportunities to enhance nursing knowledge and provide leadership in improving nursing practice.
Maintain and update professional certifications as required by policy.
Participate in safety inspections and report unsafe working conditions to supervisors.
Collaborate with NM and other VA leadership to ensure compliance with Joint Commission, OIG, and other regulatory agency guidelines.
Demonstrate a leadership role during EOC rounds and assist NM with correcting identified deficiencies.
Serve as a resource for other LPN/MT/MSA staff regarding Continued Readiness Initiatives.
Apply infection control measures appropriately in nursing care.
Demonstrate knowledge of processes for reusable medical equipment and proper setup and maintenance according to SOP.
Assist RN/LPN in preparing patients for transport to appointments, other departments, emergency department, or unit transfers.
Perform treatments and procedures within the scope of license and facility policies.
Work Schedule: Monday – Friday, 8 am – 4:30 pm.
