Position Title: Registered Nurse/Licensed Practical Nurse (Immediate Hiring for this role)
Department: Skilled Nursing Facility
Type: FT Role
Shifts: Monday–Friday and weekend availability (12 hour shifts)
Reports to: Director of Nursing

Job Details
The Registered Nurse (RN)/Licensed Practical Nurse (LPN) is responsible for the overall nursing care/service on their shift, including the accurate and timely administration of medications, treatments, documentation, and emergency calls, as indicated. This person will be the clinical partner within the household team, nurturing member-centered and member-directed culture. The RN/LPN serves as the day-to-day manager for the CNAs and CMAs on their shift and becomes the role model of professional nursing practices for the CNAs, CMAs, members, families and other team members.
Essential Duties
Supervise direct care of members.
Supervises the Certified Medication Assistants and Certified Nursing Aides to ensure timely and appropriate medications are administered.
Provides direct care as necessary.
Provides and receives inter‑shift reports timely and efficiently to ensure good communication about the needs of members and the community.
Rounds twice a shift to ensure proper care is rendered in a safe and clean environment.
Keeps physicians and families aware of changes in member’s condition.
Participates in health teaching and counseling to promote and maintain optimum health of member.
Maintains accurate documentation of all care provided.
Transcribes orders as necessary.
Reviews all medications for availability, expiration and proper documentation and notifies the appropriate guide if discrepancies are noted.
Appropriately destroys and documents any expired medication per established policy and procedure.
Works with members, family, and pharmacy to maintain adequate medication levels.
Maintains the self‑respect, personal dignity, and physical safety of each member.
Responds to independent living member emergency calls if indicated.
Participates in family/member counseling as necessary.
Completes Medication Administration Record (MAR) to cart audit weekly.
Completes chart checks weekly to ensure compliance with written physician orders.
Completes month‑end reconciliation with guide input.
Monthly nursing evaluation.
Required Basic Qualifications
RN/LPN is currently licensed by the State of Georgia.
At least 6 months of nursing experience with geriatric involvement preferable.
Must be able to work in self‑directed work groups.
Passion for working with elderly people.
Physical Requirements / Working Conditions
Ability to lift, move, and position patients safely.
Ability to stand for extended periods and operate medical equipment.
Ability to respond quickly and efficiently during emergencies.
May be exposed to biological hazards or clinical environments.
