Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on‑site support within one or more partnered long‑term care facilities in the Atlanta, Georgia area, with a primary focus on the Dunwoody vicinity. This role is an integral, in‑person member of our clinical care team, working alongside Ennoble Care providers in a family‑practice‑style model of care that emphasizes continuity, relationship‑building, and patient‑centered support.

Key Responsibilities
Coordinate care for long‑term care residents using Ennoble Care’s EMR
Maintain regular, proactive, on‑site engagement with residents to support care plans, follow‑ups, and care transitions
Collaborate closely with Ennoble Care providers within the long‑term care setting
Serve as a key liaison between providers, facility staff, residents, and families
Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner
Accurately document patient interactions and care coordination activities
Follow established workflows, documentation standards, and escalation protocols
Contribute positively to team collaboration and strong facility partnerships
What Embedded Means at Ennoble Care
This role is not remote or call‑center. As an Embedded Care Coordinator, you are physically on‑site in one or more partnered long‑term care facilities, working alongside providers and facility staff. You build consistent relationships with residents and families and become a trusted, familiar presence within the facility – supporting care the way a traditional family practice would, but in the long‑term care setting. The work is very much family‑practice focused, including a strong emphasis on remote patient monitoring and proactive care coordination.
Requirements
Legally authorized to work in the United States of America
Active, unrestricted clinical certification or licensure, such as Medical Assistant (MA) / Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), Registered Nurse (RN)
Minimum 1 year of patient‑facing experience in a long‑term care, skilled nursing, or similar clinical setting
Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person
Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long‑term care setting
Well‑developed organizational and time‑management skills, paired with an optimistic approach and initiative‑driven mindset
Strong working knowledge of healthcare terminology and care coordination principles
Proficiency with Microsoft 365, including Outlook, Word, and Excel
Ability to work full‑time, Monday through Friday, 8:30 a.m. to 5:00 p.m. EST (or thereabouts, contingent on facility hours) in an on‑site role, based in the Atlanta, Georgia area, with a primary focus on the Dunwoody vicinity
No on‑call
Daytime hours
Seven company‑paid holidays annually
Transportation requirements: valid, unencumbered driver’s license; consistent, reliable transportation; current auto insurance, with the ability to provide proof; travel between facilities may be required
Ability to pass a background check and drug screen
