Job Summary
We are seeking a Nurse to join our team. In this role, you will primarily provide direct care to IDD children in home settings.
Responsibilities Clinical & Nursing Duties
Administer medications per physician orders and agency policy.
Monitor for side effects, adverse reactions, and medication effectiveness.
Conduct routine health assessments (vital signs, general health status).
Provide chronic condition monitoring (e.g., diabetes, seizure disorders).
Document all nursing interventions in accordance with NC Medicaid and agency standards.
Maintain accurate Medication Administration Records (MARs).
Medication Management
Ensure proper storage, labeling, and documentation of medications.
Perform medication counts and reconciliation.
Train and oversee DSPs on medication administration (if applicable).
Report medication errors per incident reporting protocol.
IDD & Behavioral Health Support
Collaborate with ABA, clinical, and direct care staff.
Support implementation of Behavior Support Plans (BSP/BIP).
Monitor for behavioral changes related to medical conditions or medications.
Provide education to staff and caregivers on:
Health conditions
Medication protocols
Safety and risk factors
Community & In-Home Services
Deliver nursing services in:
Client homes
Community settings
Day programs (if applicable)
Promote independence and skill-building consistent with person-centered plans.
Minimum Qualifications
Active Licensed Practical Nurse (LPN) license (state of practice).
Minimum 1 year experience preferred in:
IDD
Behavioral health
Community-based services
Knowledge of:
Medication administration
Developmental disabilities
Behavioral health conditions
Performance Expectations
Maintain compliance with documentation standards.
Ensure medication accuracy and safety.
Support positive health and behavioral outcomes.
Demonstrate professionalism and ethical practice.
The Licensed Practical Nurse (LPN) provides direct nursing care, medication management, and health monitoring for individuals with Intellectual and Developmental Disabilities (IDD) receiving services in home and community-based settings. The LPN supports person-centered care, ensures medical and behavioral health coordination, and promotes health, safety, and independence.
Currently licensed Registered Nurse (LPN) in state of employment. Previous experience as a Registered Nurse is preferred. Excellent interpersonal and client care skills. Valid driver’s license.
First aid/CPR certification required.
