Occupational Therapist (OT) - Home Health
Location: Laconia, New Hampshire – Position Type: Part-Time or Full-Time
Sign on bonus: $5,000 ($2,500 paid at 90 days and $2,500 paid at 6 months)

What You’ll Do
Provide in-home occupational therapy services to patients and, when applicable, in assisted living settings
Evaluate patients’ physical, cognitive, and environmental needs
Develop and implement individualized care plans in collaboration with the healthcare team
Help patients improve daily living skills, mobility, and overall independence
Recommend adaptive equipment and home modifications to enhance safety
Educate patients and caregivers on treatment plans, exercises, and long‑term wellness strategies
Document patient progress and adjust care plans as needed
What We’re Looking For
Active Occupational Therapist license in the State of New Hampshire
Graduate of an accredited Occupational Therapy program
Minimum of 2 years of clinical experience (home health preferred)
Current CPR certification
Strong clinical judgment, communication, and interpersonal skills
Ability to work independently and manage a flexible schedule
