Overview
Desert Valley Medical Group, affiliated with Desert Valley Hospital, offers a Case Manager position in an acute care setting. The role focuses on reviewing patient medical information to assess, plan, and coordinate cost‑effective care for capitated members at significant medical risk.

Responsibilities

Coordinate interviews and review medical records as assigned to caseload.

Actively participate in case management and committee team meetings.

Analyze patient medical information from records and other sources to assess and plan care for PMG/IPA capitated members in ambulatory and institutional settings.

Coordinate discharge referrals and collaborate with clinical staff, fiscal intermediaries, patients, families, interdisciplinary care teams, and other committees.

Manage continuity and transition of care, including behavioral health and community resources.

Qualifications

Knowledge of Utilization Management/Case Management terminology and functions in managed and non‑managed care environments.

At least one year of experience as a case manager in an acute care setting or related field, including coding chart review.

Basic to intermediate computer knowledge.

Valid RN/LVN California license.

Pay Transparency
Competitive compensation is estimated between $23.37 and $30.29 per hour. Exact starting pay will be determined at hiring time based on qualifications and experience.
